At about 6:17 p.m., a woman in her 90s entered I-15 at Exit 5 and drove south in the northbound lanes. She was driving a 2017 Cadillac sedan. Near milepost 4, the Cadillac struck a 2019 Toyota Highlander head-on.

The Cadillac driver, who was the only occupant, died at the scene. The Toyota carried four people. A female driver and a male front seat passenger, both in their 70s, also died at the scene. Northbound I-15 closed at MP 4 while responders worked the scene.
